We (BEMC) ask that you read over the following information. If you agree to the terms and conditions, please sign and submit the form below by Friday, June 2, 2023.

I/we agree that the funding provided will be used for the following grant proposal: I/we agree that the funding provided will be used for the following grant proposal:

I/we agree that upon implementation/completion of this project, that I/we will provide Brunswick Electric Membership Corporation with a final evaluation of my grant-funded project. The evaluation may be written, or in the form of a PowerPoint presentation. It may include photos, video or whatever media is best to show the full breadth of the project and its impact on the community. I agree to send the final evaluation back no later than December 30, 2023. Please let us know if you need more time to complete your written evaluation.

I/we agree that any publicity for this project will include the name of the sponsoring electric cooperative, Brunswick Electric Membership Corporation.

I/we agree that should my project status change Brunswick Electric Membership Corporation will be notified immediately and funding may need to be returned to BEMC.

I understand that failure to submit my evaluation may result in automatic exclusion from future grant consideration.
